If you are a veterinarian referring a patient to our internal medicine service, please complete the referral form below. Providing relevant patient and case information helps our team review the referral and prepare for the patient’s care.
Internal Medicine Referrals
Our internal medicine service evaluates and manages a range of complex conditions, including urinary and kidney disease, blood disorders, endocrine disorders, respiratory disease, infectious disease, gastrointestinal disease, autoimmune conditions, and chronic upper respiratory or nasal disease.
Our board-certified internal medicine specialist works with referring veterinarians to review patient history, evaluate diagnostic findings, and determine an appropriate plan for further evaluation and treatment.
Submit a Patient Referral
This form must be completed and submitted by the patient’s primary care veterinarian.
